    School Entrance Requirements
     
    Students must have a physical done within the year of enrollment into an elementary school
     
    Minimum Immunization Requirements - List of Vaccines required to attend school
     
    Religious Exemption - Form to be completed by parent and notorized for religious
                                            exemption for immunizations
     
    Medical Exemption - Form to be completed by Dr. for a medical exemption for
                                         immunizations
     
    School Entrance Physical Form - Form completed by Dr. for entrance into school
