School Entrance:

It is the a law of Virginia that students may not enter elementary school without a physical done within the last year. For middle and high school, we must have a copy of the student's physical when he/she entered in the cumulative record.

          Physical form:  MCH 213 G (2014) Physcial Examination Form

Immunization:  List of Requirements

Exemptions: Certificate of Religious Exemption Form, Medical Exemption Form

Medical/Health Care Forms:

        Medication Form: Required to administer medication at school

  Physician In put Form : Used to develop health care plan addressing medical needs

  Release of Information Form: Grants permission for staff to contact provider to clarify info.

  Food allergy Action Plan Completed by Dr. for medical response to food allergies.

        Seizure Action plan: Completed by Dr. for medical response to seizures.

        Asthma Action Plan: Completed by Dr. for medical response to asthma emergencies
