If at all possible, Parents/Guardians are advised to give medications at home.
Students will be given medication at school when the medication is prescribed or ordered by a physician, after a parent/guardian and the physician completes a Medication Permission Form, and if the medication must be given during school hours to maintain sufficient health to participate in the school program.
Narcotics will not be given at school.
Failure to adhere to the prescription and non-prescription (over the counter) medication regulation may be considered a violation of the substance abuse policy.

Parents should also sign an exchange of information form as well. The nurses use this to obtain documentation from the physician for medication changes, etc. If you have any questions, please do not hesitate to call the clinic aid Kim Baska, or the Regional Coordinator Sherry Harvey at the high school.
Release of Information Form: Used to fax to physician for medication changes. (Valley Pediatric Patients will need to complete the Valley Pediatric Exchange form at their office)
