Important for Meeting Student Medical Needs at School 

Physician Input Form - Completed by doctor in order to address student medical needs at school

Medication Permission Form - Completed by a doctor and signed by a parent in order to take medication at school

Release of Information Form - Signed by parent granting permission for school staff to exchange information with a medical provider to meet medical needs at schools.  (Valley Pediatric Patients will need to complete the Valley Pediatric Exchange form at their office)

Food Allergy Action Plan - Completed by a doctor and signed by a parent to aid students with Food Allergies. 

Asthma Action Plan - Completed by a doctor and signed by a parent to aid students with asthma

Seizure Action Plan - Completed by a doctor and signed by a parent to aid students with seizures

Certificate of Religious Exemption - Signed and notarized certificate affirming the administration of immunizing agents conflicts with the student's/my religious tenets or practices.

Medical Exemption Form - A signed certification from a licensed physician or local health department that one or more of the required immunizations may be detrimental to the student’s health.
